Program Features

H.E.A.R.T. Homes are a unique choice for elders seeking 24-hour residential care in a non-institutional environment. We feature affordable family-style living enabling elders to age in place, while offering assistance with personal care, laundry, medication reminders, shopping, cooking, cleaning and other activities of daily living.

Our residents sign a lease for his or her bedroom and the shared use of the common areas including full kitchen, dining room, family room and yard.

Program Costs

The cost of residency in one of our homes is split between occupancy and service fees. The occupancy fee includes:

• Rent
• Groceries
• Utilities, Telephone, Cable TV
• Heat, Electricity, Water
• Landscaping, Snow Removal
• Home Maintenance

The basic service fee includes 24-hour staffing and services based on each individual resident's care plan. Additional services such as nursing visits and transportation can be arranged through Elder Home Options at pre-arranged costs. Our residents pay either privately or have their services provided by available state and federal programs.

Resident Criteria:

• Age 55 or older
• Non-smoker
• Willing to accept services at home
• Desire to live with others
• Requires some assistance with daily activities
